Health Care Coordinator (HCC)

Job Summary:

Responsible for assigned individuals; whereby, assisting, teaching, and caring for their needs as necessary for their daily functioning.  To understand and implement the individual habilitation plan of each individual served in the community residential facilities.  Responsible for assuring individual’s needs are being met and they are receiving support and services enabling them achieve their goals and objectives as defined in their individual plan.  Understand the responsibility for assisting individual clients in reaching his/her highest potential in every aspect of their lives in the least restrictive environment, respecting their privacy, and dignity.

Qualifications

  • A minimum of 18 years of age
  • A clear criminal record (no felony, violent misdemeanor, or active warrant of any kind)
  • A minimum of a high school diploma or GED equivalent
  • A valid Oklahoma Driver’s License with a good driving record
  • Documentation of personal vehicle insurance and reliable transportation
  • Experience working directly with individuals with intellectual and developmental disabilities
  • Sufficient maturity to organize and lead effectively
  • The ability to accept responsibility and act independently in performance of job duties

Major Duties

  • Serves as Health Care Coordinator (HCC) for assigned individuals to ensure implementation and coordination of health care services.  The HS/HCC maintains an overview of the individuals’ health care needs and lifestyle, is trained by DDS and/or contract staff, documents that health concerns are acted upon, monitored, communicated and other staff working with the individual are trained in person specific health supports and monitoring.  The HS/HCC accompanies assigned individuals to health appointments and assists the individual in communicating fully with health care providers.  The HS/HCC keeps identified health care providers advised of medical status and data regarding any target symptoms as well as communicating medical/dental orders with Team Members and other service providers as appropriate.  The HS/HCC prepares and presents the Referral Form for Examination and Treatment to the health care provider at the time of the encounter and requests a short written summary of the findings/recommendations.  The HS/HCC works with HTS backups or designee’s as needed to complete all tasks.
  • The HS/HCC is responsible for assuring identified self-help trainings (i.e.; toileting, personal hygiene, dental hygiene, dining, bathing, dressing, grooming, communication, home/room maintenance, communication, mobility, etc.) are provided as needed.  The HS/HCC is responsible for ensuring that safe, quality care is provided in accordance with regulatory requirements.  This includes ongoing environmental monitoring and reporting of  needed repairs/maintenance for action are being completed, providing individual and home specific training to any HTS working with assigned individuals (i.e. such as but not limited to meal preparation, kitchen cleaning, laundry, safety monitoring and individual’s clothing care), and promoting the individual’s participation, involvement, and self direction of daily life activities.
  • The HS/HCC is responsible for; supporting and promoting individual’s daily problem solving skills enabling self management of behavior, implementing behavior support plans and related policies, acquiring and maintaining required training/certification(s), providing individuals with adequate information to promote choices for leisure activities and providing training and assistance to individuals in managing their personal funds following established policies and procedures.
  • The HS/HCC is accountable for assuring the safety, dignity, and confidentiality of all individuals.  This includes maintaining service to the clients, working with other team members in the development and implementation of Individual Plan programs, maintaining an ongoing awareness and knowledge of Individual Plan goals and objectives, providing related training and interaction to support a meaningful day, achievement of outcomes, and coordinating emergency and evacuation drills.  The HS/HCC is accountable for working a minimum of a 40 hour workweek (staffing patterns will vary based on individual needs) and will be available for emergency coverage during episodes of staff shortage as needed.
  • The HS/HCC is responsible for assisting the assigned Program Coordinator in checking and maintaining all Individual Plans and records, assuring assigned assessments and reviews related to Individual plan development are completed and implemented in a timely manner, and reporting any suspected abuse, neglect or exploitation of individuals served according to applicable law and agency policies.  The HS/HCC is also responsible for completing when necessary physical intervention documentation and Accident/Incident Reports following established procedures, keeping the Program Coordinator and or the Lead Program Manager informed of staff and individual concerns or problems as they arise.  The HS/HCC demonstrates a working knowledge of applicable rules, policies, procedures and contract standards.  The HS/HCC assures time sheets, mileage claims, monthly reports and other specified documents are submitted for assigned staff in accordance-established time frames.
  • Maintain current certification as required by Gatesway and DDS and the Individual’s Plan.  Complete training required such as First Aid, CPR, MAT, Blood borne Pathogens, and all other requirements outlined in Gatesway policies and procedures, and any and all Individual Specific In-Servicing.

Candidate must have clean background check and no substantiated reports of client abuse or neglect.
